Prótesis híbrida en pacientes edéntulo total

Abstract

La presente investigación tiene por objetivo describir un tipo de tratamiento fijo en maxilas completamente edentulas, la cual recibe el nombre de prótesis hibrida. Hemos desarrollado: definición de prótesis hibrida, biomecánica, protocolos quirúrgicos, técnicas de impresión. La metodología utilizada incluye la revisión de la literatura científica. Como resultado de esta revisión podemos realizar un buen plan de tratamiento devolviendo a nuestro paciente desdentado total resultados funcionales y estéticos. Podemos clasificar la prótesis hibrida en; prótesis hibrida de baja complejidad, prótesis hibrida de mediana complejidad y prótesis hibrida de alta complejidad. Sobre biomecánica, debemos seguir una secuencia de plan te tratamiento para reducir la sobrecarga en nuestra futura prótesis hibrida, dentro de las cuales podemos mencionar: diseño de la prótesis, factores de fuerza del paciente, densidad ósea en los espacios edentulos, posiciones clave de los implantes, nùmero de implantes, tamaño de los implantes, hueso disponible en los espacios edentulos, diseño de los implantes, y evaluar si colocaremos extensiones en la prótesis. En el protocolo quirúrgico podemos mencionar técnica All On Four, sistema ITI, entre otros tratamientos. Conclusiones: un tratamiento de prótesis hibrida, es una buena elección que puede tener el paciente que sufre de edentulismo total, ya que la prótesis estará fija en la boca por la ayuda de implantes, se obtendrán resultados tanto funcionales como estéticos. Para reducir el riesgo de sobrecarga biomecánica en la prótesis a colocar, debemos realizar un buen plan de tratamiento y obtendremos como resultado el éxito de la prótesis. La posición clave de los implantes es un factor importante al colocar la prótesis hibrida. Debemos de tener en cuenta la forma de la arcada al rehabilitar, ya que si no incluimos pilares terminales en el plan de tratamiento y queremos utilizar una extensión en la prótesis, los demás factores de fuerza deben ser moderados y reducidos para poder compensar el incremento de fuerza. Tenemos que tener encuenta con que cantidad de hueso dispone el paciente en el reborde edentulo para saber cómo será nuestra rehabilitación y tener encuenta que técnica quirúrgica utilizaremos y sea más favorables para el paciente. La técnica de impresión para una prótesis hibrida, es la técnica de cubeta abierta ya que se inmovilizaran los componentes de unión de la impresión y tendremos buenos resultados.

The present research aims to describe a type of fixed treatment in fully edentulous jaws, which is called a hybrid prosthesis. We developed: definition of hybrid prosthesis, biomechanics, surgical protocols, printing techniques. The methodology used includes the review of the scientific literature. As a result of this review we can perform a good treatment plan giving our patient complete toothless functional and aesthetic results. We can classify the hybrid prosthesis into; low-complexity hybrid prostheses, medium-complexity hybrid prostheses, and high-complexity hybrid prostheses. On biomechanics, we must follow a sequence of treatment plan to reduce the overload in our future hybrid prosthesis, among which we can mention: prosthesis design, patient strength factors, bone density in edentulous spaces, key positions of the implants, number of implants, size of implants, available bone in edentulous spaces, design of implants, and evaluate if we will place the extensions in the prosthesis. In the surgical protocol we can mention All On Four technique, ITI system, among other treatments. Conclusions: a hybrid prosthesis treatment is a good choice for the patient suffering from total edentulism, since the prosthesis will be fixed in the mouth by the help of implants, it will obtain both functional and aesthetic results. To reduce the risk of biomechanical overload in the prosthesis placement, we have to make a good treatment plan that will result in a success of the prosthesis. The key position of the implants is an important factor when placing the prosthesis hybrid. We must take into account the shape of the arch when rehabilitating, since if we do not include terminal abutments in the treatment plan and want to use an extension in the prosthesis, the other force factors must be moderate and reduced in order to compensate for the increase in force. We have to consider how much bone the patient has in the edentulous ridge to know how our rehabilitation will be and consider what surgical technique we will use and be more favorable for the patient. The printing technique for a hybrid prosthesis is the open cuvette technique since the print bonding components will immobilize and have good results.
